- you will only be billed for the 20% copay and any unpaid deductible
- Most health insurance and HMO coverage accepted
- We will file up to two claims under your policy
- We ask that copayments and deductibles be paid at the time of service
- Payment is expected at the time of service for office visits not covered by a government program
- Workers' Compensation claims will be filed with your company's carrier for your convenience
- We accept cash, personal checks, Mastercard, Visa and Discover credit cards
